You’ve set up your first appointment and now all you need to do is fill out the simple forms below and bring them with you. If you prefer to have the forms sent to you by email or text, please call and let us know.
Get directions to our office
The first time you come in to our office, you will meet our team and sit down with a doctor before we even get out the dental tools. We want to get to know you and your teeth.
Keeping up with your dental health
